transfusion rate for 10 months old

Little Evi is 10 months old and weights about 7 Kg.
Doctor has advised 100 ml of washed RBC given over 5-6 hrs.

My question is how both amount and time determined ? Are we doing it too fast or too slow?

Also doctor advises us to do hemoglobin check after 1 hr of transfusion. Does it take 1 hour to get correct reading of hemoglobin after transfusion?


Can we expect similar rise of hemoglobin levels with same amount of blood transfusion (given weight remains constant)
Can we expect similar fall of hemoglobin level over time, that is, if we see hemoglobin fell by 1 gm in 2 weeks , can i expect similar rate of fall in future?

Hi,

The formula to calculate the amount of blood is 15 ml per kg,so it seems Little Evi has been given the right amount.I can't seem to remember the time rate right now may be some other member will be able to help you with that.

When our daughter started transfusing we were also asked to get her hb after the transfusion was done,but now we don't do it anymore,it was an initial practice to determine how much her hb raises and according to that her next transfusion was planned,but now that we have almost established her transfusion pattern we don't need it anymore.

About the fall of hemoglobin,it depends on so many things,like blood's age (how many days ago it was donated),and the child's health (if he has had any infection the hb will fall faster then usual) but with time being as he grows and is being transfused regularly you will be able to determine a regular interval between his transfusions,and it will also depend upon what is his pre-transfusion hb,like when our daughter's pre-transfusion hb is above 9.5,we take her next appointment for 4 weeks and if its below 9.5 we take her next appointment for three weeks.

The initial transfusion rate is 2 mL/minute for the first 15 minutes. Once it is apparent that there is no reaction, the suggested rate for transfusions is 5 ml/kg per hour, so the standard actual transfusion time for 100 ml is about 3 hours. You may have been told a longer time for two reasons. First, this estimate may include the prep time before the transfusion, and second, the rate may be slower for the initial transfusion(s). Blood should not be given at a faster rate unless it is an emergency situation involving blood loss, as the sudden introduction of a higher volume can stress the heart.

2ml/minute for first 15 mins means 30 ml in first 15 mins. Which means too fast  :huh

Did you mean 2ml per kg per hour andy?

Thanks for pointing that out, pmeet. That is a mistake in the quoted source and is actually repeated at many other sources. Yes, it should read 2 mL/kg/hour. I will be replacing our document on Blood Administration with a new one without the typo.
