Thankx for reply.

1. I am not using filters Set or filtered blood.

2. It is just phynotyped blood (Extended panel Cross Matching) Leuko Reduced RCC which has been started by me since June 2012.

3. I am advised to use filter set (by one doc) and Triple Saline Washed RCC (by another doc.) coz i am having allergic reactions on skin due to blood transfusions.

4. Should i use the Filter set or Triple Saline Washed RCC , which is good for me?

Plz guide, i want to see my pre bt hb at 9.5 to 10 gms%

A filter will remove more white cells. Since you are reacting, a filter should be tried for a few transfusions to see if your blood lasts longer.

Washed cells are a good option, but not quite as effective as a filter. Even with filters, fragments sometimes get through, causing reactions.

Just to add, I was using filter in early 2009 but eventually stopped using it, it didn't caused me any low WBC situation as I remained protected all the times, but it actually caused more troubles than any good it may do. Sometimes the blood won't just flow, maybe due to weather (cold) or some other issues, so I stopped it. And I haven't noticed any good or bad difference with or without using a filter.

I take TSW (Triple Saline Washed) PCV around 500 CC (ml) which lasts around 21 days keeping my Hb above 9 or sometimes around 9.5.

I am going to use Bayer filters on next transfusion. Pray for me that blood lasts longer. I really need higher pre bt hb.
@ Pratikdear,
You might be using TSW RBC & not PCV. Because   PCV contains White cells in addition to Red Cells. We require Red Cells only. Do check it.

Dear Andy,
There is no improvement in hb level after using filters.
Pre bt hb and post bt hb both came at 9 gm/dL, inspite having large unit of blood.
After that (in next week) pre bt come to 8.4gm/dL.
Now transfusing blood at every week(large unit and without phynotyping  using filter)

Every time after tx abdomen area become hard and thenafter in some day returned to normal.

Splenectomy to be done?

Splenectomy should be a last resort. My opinion is don't remove the spleen unless the size is a danger. How large is your spleen?

Actually it is very long time i had have sonography. Old Report on Feb 02,2010

Spleen Size about 17.5 cm.
Conclusion
Moderate spleenomegaly noted
Mild hepatomegaly noted

I will go for latest Sonography of Abdoman Area.

By the way , what are the other options to improve the hemoglobin ?

Regarding the liver pain or abnormalities,

you can try turmeric (Haldi/haldar) with honey.

I'm taking it these days due to throat infection and it also doesn't increase my sugar levels at all, instead I find it's helping me to manage my sugar levels.

When in past I took, my SGPT levels were very nice, 28 or so.

I plan to take this now at least 3-4 times a week.

Actually i am taking turmeric with Milk 365days in the morning.
My SGPT level is 32 which was staying around 65 before starting Desirox.

Just add some Ghee drops to Honey + Turmeric combination , it will work best for throat infection, in addition to it Chew Tulsi Pan daily. It's my personal experience and it works

Sonography Abdomen Screening

Conclusion:
Hepato-splenomegaly.
Spleen measures about 172mm.

Please Help. Should i go for splenectomy?

I would not suggest splenectomy yet. It is large, but not dangerously so. I would say give i some time and also talk to the doctor about the antibody reaction that cause the high rate of blood turnover. We have also previously been told of some herbal remedy for spleen size. Have you tried anything like that yet?

Antibody reaction test COOMB Test was done before 2 years and it was Negative. Before just one month it is NEGATIVE. Whether any other test require? or shoud is go for retest of COOMB's Test , Direct and Indirect?

Regarding herbal remedy the problem is that it is not used by any other. If i take it and if there is some reactions then there will be no body to rescue me. So i am a bit afraid.

Morover i am going to start Wheatgrass tablets and Natural Vitamin E and hope it will help in hb maintaining.
Since some weeks I am taking Evion 200 and i feel that it is helping very very limited but it is. I feel that Evion is avoiding chest pain and back pain happend to me before a day of Tx.

The pain is usually because of low Hb, I believe. And yes, I too take Evion (Vit E brand in India) and I feel it helps me a lot. It also helps me get away and saves me from any fatigue and restlessness.

About Hb, usually, if and when, sometimes, I've low Hb, like 8 or so (in very rare cases), my legs and joints do pain, which after TX, on the next day of it, are much relieved. So yes I do believe there is some connection of pain due to Hb levels being low.
