Hi Andy,
 My son had his transfusion today. He needs 380ml by 20 ml /kg rule.
Paediatric units are around 300ml and adult 500.
Choices are to use two Paediatric units and expose him to antigens from 2 donors or to bring him in earlier which would be2-3 weeks instead if 3-4 weeks.
This problem will continue till he is about 25 kg I guess and qualifies for the adult unit.
What is the standard and which would you recommend?

Hi Zahra,

My ideal standard would be once weekly, which of course is not practical, but you see my direction. I would prefer minimizing the amount of donors, so taking blood one week earlier would be my advice. If there is any consideration at all about pursuing a BMT at any future point, this would become even more important, as exposure to fewer antibodies means less chance of reaction to the new bone marrow.
